| A team effort is required to provide those facing life-limiting illnesses with the dignity, care, and comfort needed to experience the most joy and fulfillment possible in the time that remains. Our program is designed to allow patients to maintain as much control as possible over their life choices, for as long as possible throughout the illness process.
Each patient has an individualized plan of care. Our caring hospice staff includes nurses, home health aides, volunteers, clergy, and social workers who build trusting relationships with families while providing quality end-of-life care for their patients. LIFESPAN Hospice In-Home Care is integrated with our other LIFESPAN services and provides care for patients of all ages and backgrounds with a variety of life-limiting illnesses. |
